As a music school we like to actively promote all the great work that our students, their parents and
our staff do during their time with us. This means promoting activities and achievements using
photos and videos etc. via a range of marketing and media channels. We would like you and your
children to be part of our school story and therefore ask you to complete this form.

I (parent/guardian or adult student) give my consents to DoReWe music school to take photos and
make audio and video recordings of my child/children/me and their/my work or results and consent
that these photos and vidoes can be published and processed on:
